Midwest Championship Series – Sioux City Tournament Team Registration

I give permission, on behalf of the players and their parents, for the players to play in the Tournament(s).  I understand that Siouxland Sports Academy, The Arena Sports Academy, Sacred Hoops Basketball, the sponsoring organizations, its committee members, and volunteers have no responsibility, assumes none, and do not carry accident insurance for the benefit of players. By participating in the tournament, I as the registrar, all players, parent(s) of players, and guardians of players, Siouxland Sports Academy, The Arena Sports Academy, Sacred Hoops Basketball, the sponsoring organizations, its committee members, and volunteers from all claims of any injuries and lost or stolen property which may occur while participating in this tournament.

I acknowledge, as an event attendee, that I hold no reasonable expectation of privacy or confidentiality associated with the images and video taken or used by Sacred Hoops Basketball, The Arena Sports Academy and participating program organizations. By attending this event, I authorize the use of such images and video indefinitely without payment or any other compensation to me and without obtaining my additional consent for future uses.
